Download the MyChart Mobile App now to access your health chart from mychart.centracare.com. By downloading the My Chart Centra Care Mobile App you will be able to manage your health appointments, view your medical chart, and more, all on-the-go from your internet-connected mobile device. This could be your smart phone, tablet or other device.
There are two buttons on the mychart.centracare login page that you can use to help direct you to the proper app store according to the operating system of your device. If you have an Apple device, you can use the button that has the Apple logo and says App Store on it. This will redirect you to the download page that you will need. If you have an Android or Google device, you can use the button that says Google Play.

Some of there features are:
- Access your lab or other test results without having to wait for a phone call or letter from the lab, hospital or your doctors office. See your test results with your physician’s comments within days of having your test done.
- Manage all of your health-related appointments. Schedule your next appointments or even view details of your past and future appointments.
- Send in online requests for prescription refills instantly. This features applies to all of your refillable prescriptions.
- Communicate with your physician or health care team. This allows you to get answers to your important medical questions from the comfort of your very own home. It is important to remember, though, that this feature should never be used for emergency situations or situations that require urgent care.
If you already have an account with Centra My Care Chart, all you have to do is enter your MyChart Username and Password and hit the Sign In button. Once you do that you should be granted immediate access to your account. If you have forgotten your login information, you can use the two links labeled appropriately in order to get help recovering or resetting this information.
